Terms of Service

By using our website, scheduling appointments, or receiving services from Family Tree Dentistry, PA, you agree to be bound by these Terms of Service. If you do not agree to these terms, please do not use our services.

Scheduling and Confirmation:

• • Appointments must be scheduled in advance through our office
• • We will confirm appointments via your preferred communication method (text, call, or both)
• • It is your responsibility to keep your contact information current

Cancellation Policy:

• • At least 48 hours notice is required for appointment changes or cancellations
• • Late cancellations (less than 48 hours) may result in a $100 fee for treatment appointments
• • Late cancellations for preventive care appointments may result in a $50 fee
• • No-shows will be charged the full cancellation fee
• • More than three missed appointments may result in dismissal from the practice

Late Arrival: Patients arriving more than 10 minutes late may need to reschedule their appointment.

• • Payment is due at the time services are rendered
• • We accept cash, credit cards (Visa, Mastercard, Discover), Apple Pay, and CareCredit
• • Personal checks are not accepted
• • For treatment appointments over one hour, a reservation fee may be required
• • Insurance claims will be submitted on your behalf, but you are ultimately responsible for all charges
• • Payment plans may be available through CareCredit or other arrangements

• • Provide accurate and complete medical and dental history
• • Inform us of any changes to your health status or medications
• • Follow treatment recommendations and post-care instructions
• • Keep scheduled appointments or provide proper notice for changes
• • Pay for services as agreed
• • Treat staff and other patients with respect and courtesy

By receiving treatment, you consent to the dental procedures performed by Dr. Bui and her team. All treatment options, risks, and alternatives will be discussed with you prior to treatment. You have the right to accept or decline any recommended treatment.

We are committed to protecting your privacy and maintaining the confidentiality of your health information in accordance with HIPAA regulations and our Privacy Policy. Please refer to our Privacy Policy for detailed information about how we collect, use, and protect your information.

Our liability is limited to the cost of the dental services provided. We are not responsible for any indirect, consequential, or punitive damages. All treatment is provided according to accepted dental standards and practices.

For dental emergencies during business hours, please call our office immediately. For after-hours emergencies, please call our office number and follow the emergency instructions provided.

We reserve the right to modify these Terms of Service at any time. Changes will be posted on our website and will be effective immediately upon posting. Continued use of our services constitutes acceptance of the modified terms.
